2. Personal Data We Process

We process the following personal data: 

• First and Last Name
• Gender
• Date of birth
• Address information
• Phone number
• Email address
• Bank account number
• Information about your activities on our website
• Other personal data that you actively provide, for example through correspondence or telephone contact

We offer the option to schedule and modify appointments via an external app. The app processes the complete patient record, exclusively on behalf of Gewoon Chiropractie. A data processing agreement has been entered into with the app provider, and the data is secured in accordance with the GDPR and the WGBO.

3. Special categories of personal data

We process the following special categories of personal data:

• Medical data (health)
• BSN (Citizen Service Number), only if legally required
• Insurance data
• Data of persons under 16 years of age, only with parental/guardian consent

Our website and/or service does not intend to collect data about website visitors under the age of 16 unless they have permission from their parents or guardian. However, we cannot verify whether a visitor is over 16. We therefore advise parents to be involved in their children’s online activities to prevent data about children from being collected without parental consent. If you believe we have collected personal data about a minor without consent, please contact us at info@gewoonchiropractie.com, and we will delete this information.

4. Purposes and Legal Basis

Your personal data is processed for the following purposes:

• Performance of the treatment agreement and provision of care (medical data)

• Scheduling, recording, and managing appointments, including tracking attendance or no-shows
• Billing and administration of costs resulting from missed appointments

The processing of personal data is based on:

• Treatment data: performance of the treatment agreement (Art. 6(1)(b) GDPR, WGBO)
• Financial administration: legal obligation (Art. 6(1)(c) GDPR)
• Marketing/newsletter: consent (Art. 6(1)(a) GDPR)
• Website analytics: legitimate interest (Art. 6(1)(f) GDPR)
• Appointment administration and any no-show fees: performance of the agreement (Art. 6(1)(b) GDPR)

When processing based on legitimate interest, we balance our interest in a well-functioning and secure website against the privacy interests of visitors. In doing so, personal data is anonymized as much as possible.

5. Automated Decision-Making

We do not make decisions based on automated processing regarding matters that may have (significant) consequences for individuals. This refers to decisions made by computer programs or systems without human intervention.

6. Retention Periods

We do not retain your personal data for longer than is strictly necessary to achieve the purposes for which your data is collected. Your patient record is retained for 20 years after your last treatment, in accordance with the WGBO. Administrative data, such as invoices and appointment history, is retained for 7 years in accordance with tax legislation.

7. Sharing with third parties

We share personal data only if necessary for the performance of the treatment agreement or to comply with legal obligations. This also applies to the external app in which the complete patient file is maintained. A data processing agreement has been concluded with all third parties.

Personal data is not transferred to countries outside the European Economic Area, unless this is necessary for the provision of services and appropriate safeguards have been put in place in accordance with the GDPR.

8. Cookies

We use cookies as follows:

• Functional cookies to ensure the website works properly
• Analytical cookies, anonymized, for website optimization
• Tracking cookies only after explicit consent

To protect your identity, we ask that you include a copy of your ID (passport photo, MRZ, passport number, and Social Security number blacked out). We will respond within four weeks.
